(HMO) Health Maintenance Organization: Insured selects a (PCP) primary Care Doctor and obtain medical services from that PCP within the network of providers. Referrals to see specialist are needed. Physician reimbursement is fee for services rendered out of network are not covered.
Indemnity Plan: An indemnity plan is what all health insurance was before managed care came about. It has none of the bells & whistles that managed care plans offers. Indemnity Policies insure you against accidents and illness and are clearly defined within the policy as to what is covered and what is not covered. Indemnity plans cover medical expenses but a deductible will need to be met and them the insured will own their share of the medical expenses in the form of Co-insurance (example 80% / 20% the insurance will pay 80% of the bill and you the insured will pay 20% up to the contracts out of pocket maximum.
PPO Preferred Provider Organization: PPOs do not require physician referrals to see specialist in or out of network. PPO plans depending on plan design typically have rich In-network benefits similar to a HMO. Benefits obtained Out of Network are subject to a higher deductible and co-insurance.
(POS) Point Of Service: This plan acts like a HMO for In network benefits, but provides insured the option to self-refer In & Out of network. Benefits for self-referral are subject to a deductible and co-insurance. If insured obtains a referral from their (PCP) primary care Physician benefits are paid as In network.
